Research objectives and hypothesis/research questions:
The aim of this article is to present coordinated care as a model of healthcare delivery that enhances the quality of health service provision. The article addresses the following research questions: 1. How have the concepts of healthcare quality evolved? 2. What is the difference between coordinated care and integrated care? 3. How does coordinated care function in Poland? 4. To what extent does coordinated care represent an innovative approach to healthcare? 5. In which areas of healthcare does coordinated care serve as a model for improving the quality of service provision?

Research methods:
To achieve this objective, the following research methods were employed: analysis of domestic and international literature of 2005-2025 period, industry reports, and websites presenting statistical data on the aforementioned topics. The analysis presented is based on the latest studies and reports from 2023-2025, emphasizing the importance of a holistic, patient-centered approach as the foundation for new quality in healthcare.

Main results:
The implementation of coordinated healthcare in Poland represents a significant step in the evolution of the primary healthcare system and an improvement in the quality of healthcare services. Key achievements realized through the introduction of coordinated care into the Polish healthcare system include: 1. transition from a reactive to a proactive and coordinated approach to healthcare, with a clear emphasis on prevention, education, and patient activation; 2. expansion of primary care physician (PCP) competencies: The scope of diagnostics available in primary care has been increased, and care has been focused on the most common chronic diseases, aiming to relieve the burden on specialist care; 3. increased access to specialist doctors and ensuring continuity of care for chronically ill patients; 4. inclusion of the patient in the treatment process through their co-responsibility for achieving goals set in the Individualized Plan of Medical Care (IPOM) and by providing them with health education to help manage their own illness.

Implications for theory and practice:
The implementation of coordinated care opens up opportunities for further streamlining healthcare processes and enhancing the quality of healthcare services in Poland. It enables a holistic approach to treatment, strengthening the continuity and coherence of diagnostic and therapeutic processes. As the first stage of integrated care, it creates conditions for better population health management, more effective treatment of chronic diseases, and increased patient satisfaction and cost-effectiveness of the entire healthcare system.
